The PatientPIN service is intended for RadCard patients who are unable to remember their PIN.

Patient PIN allows you to review the information on RadCard.
When your medical or imaging data was loaded onto the RadCard, you would have been advised of your own personal patient PIN.
This would have been provided to you on a printed slip of paper, and may have been sent to you by SMS or email.
If you are unable to find your PIN, you can recover it by following the steps below.
